RECIPIENT RIGHTS & GRIEVANCE POLICY
Horizon Rehabiliation Center, INC., Mental Health Rehabilitation Services Program acknowledges the dignity and will protect the rights of recipients served as well as their families. This agency will ensure that each recipient has a Freedom of Choice with regard to selecting providers of services, including this one. All participation in the program is voluntary and no recipient will be forced to receive services for which he or she is eligible.
Horizon Mental Health Rehabilitation Services Program operates in accordance with Title VI and VII of the Civil Rights Act of 1962, as amended, and the Vietnam Veterans Readjustment Act of 1974 and all requirements imposed by or pursuant to the regulation of the U. S. Department of Health and Human Service. This means that individuals are accepted and that all services and facilities (waiting room, restrooms, and etc.) are available. A recipient is not denied admission segregated in to programs, or otherwise subjected to discrimination on the basis of race, color, religion, age, sex, gender, national origin, physical abilities, political beliefs, or any non-merit factor, in compliance with all state and federal regulations.
Each recipient by law, no information written or verbal, concerning the recipient or his/her family shall be released or requested with a signed, dated and witnessed statement made by the family shall be released or requested with a signed, dated and witnessed statement made by the recipient or his/her agent, authorizing Horizon Rehabilitation Center to do so. The statement of authorization shall indicate, by whom and from whom information will be transmitted and for what purposes.
As the recipient, you have the right to voice a grievance against the manner in which you are treated without fear of reprisals. When doing so, you should follow the chain of command. For example, if you have a problem with a staff member working with you or your family, you should first discuss it with that person. If the result is not satisfactory, address the problems to the attention of their supervisor and/ or program director.
